Outreach Services

Provides the following:

Assists those supported with life skills such as personal hygiene, dressing, meal preparation, etc.

Community opportunities to participate in activities that are meaningful to the person being supported.

Organized group activities offered throughout the month that promotes community inclusion as well as an opportunity to build healthy social relationships with others.

Person CenteredProgramming

Provides the following: 

  • A plan that focuses on the individuals dreams and goals in life that is built on their abilities and strengths.

  • The plan will help provide clarity and accountability for the individual and their support team to ensure everyone knows their role and responsibility.

  • A written or visual plan will be developed and reviewed regularly with the person being supported and those important to the person to ensure goals are being met. 

Day Services andSupports

Provides the following: 

  • Opportunities for those supported to develop hands-on skill building techniques that cannot otherwise be done in a 1:1 setting.

  • Friendships, modelling appropriate social skills, and mentor-ship will be done naturally as individuals develop skills together and learn from each other.

RespiteCare

Provides the following:


  • In/out of the home respite care for primary caregivers that promotes healthy family relationships.

  • Overnight Camps (in a group setting)

  • Overnight Respite options available.

Respite


Provides the following:


  • ALL accommodations included

  • 24 hour staffing support

  • Activities throughout the day (within the community or at PCC Day Services) 

AdministrativeSupports

Provides the following:

  • Brokerage

  • Bookkeeping

  • Payroll

  • Scheduling support workers, etc is also available through Person Centered Care upon request.

Coping Skills Development

Provides the following: 

  • 1:1 sessions that is focused on the development of coping skills and problem solving techniques
